If you are scheduling an appointment for someone other than yourself such as a family member, friend or minor, please make sure you are logged out of your account and create an account for that person with their full name and contact information.

 

A card on file and deposit is required to reserve an appointment. The person's card placed on file is the one responsible for paying any cancelation or no show fees as per our cancelation policy.

Make sure you are logged out of Boulevard booking before booking someone else's appointment.

For minor's, a parent or guardian must book the appointment and fill out any forms for that minor.
